This app was very user-friendly. I cruised through the easy-to-understand forms quickly.

This was the most frustrating app I've ever used to fill out medical paperwork. It took a total of 3 attempts to fill out 16 pages of information. The first of which I had filled out 10 pages and all the information was deleted. I was so angry. Also, while I filling out the forms my answers would randomly be changed or deleted. Some sections you would rate by a number scale indicated by "bubbles". The answers continued changing on their own. I was almost in tears! Horrible and frustrating!!!

I had to fill out the document I was using it for, twice. The first form filled out got lost somewhere in digital space, apparently, and I was never sent and emailed a copy of the form I filled out, which, I made sure that feature was ON. There is not a feature on the app to go back to a copy of the completed form to make sure it was sent correctly. So I have no means on the app to save or check back on the form I completed since the app did not send a copy to my email or save it in any way.
