My Horizon NJ Health TC began on 1/22. My nurse visits every 3 months to discuss changes. At 6 months, we readdress my goals making sure they're realistic & revise whatever is necessary. Finding specialists & a new family doctor was hard, but we put a great team together. MS, AVN, cancer, diabetes, osteomyelitis, chronic pain, TA, migraines & more, my immunity isn't good but with a good medical team, my nurse & my caregiver I thank & love my God for life, faith, trust & hope.

It was working fine until it was update. The app now forces close every time I try to log in. Will be Uninstaller from my phone. This app is trash.
